Please note: We have a new, improved skin prick test referral form.

It is located on our website on our ‘Providers’ tab, ‘Form’s page, entitled ‘Skin Prick Testing – Request Form’.

This new form can be completed electronically, reducing errors that often occur with handwritten forms. We will need you to complete the form, then print, sign and email to us please at immunology@pathlab.co.nz
